SimonMed is alerting potentially affected individuals after confirming unauthorized access to its network earlier this year.


SimonMed Imaging has announced it is notifying patients and regulators of a data breach that may have exposed sensitive personal and health information earlier this year.

According to the Arizona-based imaging provider, the breach was discovered after one of its vendors reported they were experiencing a security incident on Jan 27, 2025. In response, SimonMed reviewed its own systems and identified suspicious activity the following day. The company has since confirmed unauthorized access occurred between Jan 21 and Feb 5, 2025.

SimonMed said it took immediate steps to contain the breach and launched an investigation with the help of cybersecurity and data privacy experts. Mitigation efforts included resetting passwords, enhancing multifactor authentication, implementing endpoint detection and response monitoring, cutting off vendor system access, and notifying law enforcement.

While the investigation remains ongoing, SimonMed reports that data potentially affected includes patient names, addresses, birth dates, dates of service, provider names, medical record and patient numbers, diagnosis and treatment details, medication information, health insurance data, and driver’s license numbers.

Because the full scope of the breach is still being determined, the company is providing notice to all individuals who may have been impacted, according to a release from the company.

SimonMed is urging potentially affected patients to monitor their financial accounts and health insurance statements for suspicious activity. Individuals are also encouraged to request free credit reports from Equifax, Experian, and TransUnion and consider placing fraud alerts or security freezes on their credit files.
