The system integrates CT imaging and uses a fixed radiation beam, a design intended to lower infrastructure costs and expand access to care.
Leo Cancer Care has introduced Grace, an upright photon therapy system, at the American Society for Radiation Oncology 2025 annual meeting. The system— currently under development and not cleared or approved for sale—will deliver conventional X-ray radiation therapy while the patient is in a seated, upright position, which research suggests may improve anatomical consistency and organ stability compared to traditional supine treatments.
From a technical standpoint, the Grace system will integrate a fan beam CT scanner at the treatment isocenter. This feature allows for planning-quality imaging during the treatment workflow and is designed to support future online adaptive radiotherapy. The system will also include a large, high-speed multi-leaf collimator (MLC). By utilizing a fixed radiation beam, the design aims to reduce primary shielding requirements and lower infrastructure costs, potentially making installation less complex and more affordable for healthcare facilities.

And that purpose is to put the patient truly back at the center of their treatments.”
The system will be compatible with RaySearch Laboratories’ RayStation treatment planning system and the RayCare oncology information system, which now incorporate support for upright radiation therapy.
The first five pre-commercial Grace systems are slated for installation at member institutions of the Upright Photon Alliance, a global research collaborative. Alliance members, including Mayo Clinic, Cone Health, and OncoRay, will investigate the technology’s clinical potential.
